Becoming an ILiSaP Scholar

1. Year I applications open on Monday, August 4, 2003. The deadline for applications is Friday, September 5, 2003. Applications may be sent via fax (787)-758-2783 or mailed to ILiSaP PR, UPR Graduate School of Public Health, P. O. Box 365067, San Juan, Puerto Rico 00936-5067. Selections will be announced by September 22, 2003.

2. Applicants must apply as teams consisting of three to four individuals. Each team member must submit a separate application form with a complete biosketch.

3. At least one member of each team must be a top or intermediate managerial level position in his/her organization. Applicants must have a minimum preparation of a bachelor degree and at least five years experience in public health practice. Each member must make a commitment to full participation in the two-year Institute Program.

5. The cost of the full 2 year programs is $5,000 per member of a team payable to the University of Puerto Rico in the first 10 days following the acceptance notification.
